2. Ultrasound- or CT-Guided Liver Biopsy — Precision Sampling

Real-time imaging allows continuous visualization of the needle path. It is widely available, avoids radiation, and is the first choice for most focal lesions.

CT-guided biopsy

3. Transjugular Liver Biopsy (TJLB) — For High-Risk Patients

TJLB is preferred when bleeding risk is high. Instead of puncturing the liver from outside, the biopsy is performed from within the venous system.

How it is performed
Under local anaesthesia and sedation, a catheter is inserted into the right internal jugular vein, advanced through the superior vena cava into the hepatic vein. Hepatic venous pressure gradient (HVPG) can be measured simultaneously if needed.

What to Expect: Before, During, and After

Before the procedure

  • Blood tests: haemoglobin, platelet count, INR/PT
  • Temporary discontinuation of anticoagulants/antiplatelets as advised
  • Fasting for 6–8 hours
  • Informed consent after discussion of risks and benefits

During the procedure

  • Local anaesthesia (with or without mild sedation)
  • Duration: 15–45 minutes depending on technique
  • One or more core samples obtained (typically 1–2 cm in length)

After the procedure

  • Observation for 4–6 hours (longer if high risk)
  • Mild pain at the biopsy site or right shoulder may occur
  • Avoid driving the same day
  • Results usually available within 3–7 working days

Are There Risks?

Liver biopsy is generally safe when performed in experienced centers with proper patient selection.

  • Bleeding: the most important complication; major bleeding occurs in <1% of cases
  • Pain: usually mild and self-limiting
  • Injury to adjacent organs: rare, minimized with imaging guidance
  • Infection: very uncommon
  • Bile leak: rare
